HEALTH & SOCIAL CARE FORUM
Formed in 2002, the Health & Social Care Forum is a network of third sector organisations with a common interest in health, social care and well-being services within the County Borough of Merthyr Tydfil.
The Forum has direct links to the Borough’s Health, Social Care & Well-Being Strategic Partnership, which has developed the Health, Social Care & Well-Being Strategy 2008-2011. The strategy seeks to:
“improve the health and wellbeing of the population of Merthyr Tydfil through targeted and co-ordinated activity, developed in partnership, and based on evidenced good practice….”

TERMS OF REFERENCE
The Forum has agreed terms of reference which are reviewed at least every two years. The next review will be undertaken in 2009.

MEMBERSHIP
The current membership includes a broad range of Third Sector organisations operating locally, regionally and nationally.
Third sector organisations participating in the Forum are encouraged to become members of VAMT, and can register by downloading and completing an application form here.
Statutory partners are also welcome to engage with the Forum for the benefit of information sharing, consultation and networking.

CHAIR, CHAMPION AND REPS
Chair
The Chair of the Forum is elected to serve for a term of two years, as outlined in the terms of reference.
The Chair of the Forum is automatically a member of the Core Group for the Health, Social Care & Well-Being Strategic Partnership.
Champion
There are identified Champions for each of the key priorities within the Health, Social Care & Well-Being Strategy 2008-2011.
The Champions will ensure that the strategy is both developed and also delivered.
Reps
The Forum is seeking to build a pool of representatives to engage in Borough-wide planning groups and fora. To see an outline of the role and commitment required click here...
