CHRONIC CONDITIONS MANAGEMENT (CCM) FOCUS GROUP

The CCM Focus Group was formed in the autumn of 2008 to inform service planning across Merthyr Tydfil and Rhondda Cynon Taff.  It is a recognised sub group of the Health & Social Care Forum.

TERMS OF REFERENCE

The Focus Group has agreed formal terms of reference which can be found here....

MEMBERSHIP

Membership is open to Third Sector organisations operating locally, regionally and nationally. 
Third sector organisations participating in the Forum are encouraged to become members of VAMT, and can register by downloading and completing an application form here.

Statutory partners are also welcome to engage with the Focus Group in the interests of partnership working.

CHAIR, CHAMPION AND REPS

Chair

The Chair of the Focus Group is elected to serve for a term of one year, as outlined in the terms of reference.

The Chair of the Group is not yet appointed.

Champion

The Health, Social Care & Well-Being Strategy 2008-2011 has identified Chronic Conditions as one of its key priorities, and the Director of Health Strategy at the Local Health Board is the identified Champion for this priority.

Reps

The Forum is seeking to build a pool of representatives to engage in Borough-wide planning groups and fora.  To see an outline of the role and commitment required click here...
